Fun Times!

FUTURES LOVES to give back and get involved in the disability community. Each year, FUTURES co-sponsors a camp out in September with other providers in the State.  Each year, CEO Florence Hobby and FUTURES Case Managers pitch in together to host three days of fun camping out and doing fun activities for individuals with disabilities! There is face painting, movies on the big screen, games, etc! This event is anticipated by all each year.

Getting Involved: 

FUTURES enjoyed attending the National Down Syndrome Walk in 2016 in West Lafayette, Indiana. FUTURES enjoys supporting great causes and looks forward to more in the future! 

FUTURES routinely gathers supplies for those in need. Each month, Case Managers come together to identify a family, individual, or organization in need. In the past, FUTURES has supported foster children, a Respite home, a family in need, and individuals moving out into their own homes for the first time

​FUTURES provides quality Case Management services to individuals on the Community Integration Waiver, the Family Supports Waiver and the Aged and Disabled Waiver. FUTURES provides the following services:

  • Continued advocacy in all aspects of the lives of the individuals served. 

  • Annual planning and completion for Individualized Support Plans and Cost Comparison Budgets.
  • The completion of necessary screening assessments to ensure that all individuals meet the level of care required for their waiver.
  • Financial, health, and safety monitoring to ensure the well-being of all individuals served. 
  • Ongoing waiver maintenance including managing all potential changes in personal or health information and also changes in waiver service hours or waiver providers. 
  • Case Managers also remain a strong resource for individuals served, along with their families, for any inquiries they might have to accommodate and support their loved one. ​
